Receiving Outside Records
The Outside Records tab is available to all users who have access to the patient information MPage in Powerchart.

•This tab will show any outside documents already available for the patient, such as direct message CCDs saved to the chart and any patient clipboard documents that have been sent in from the patient portal.
On the Outside Documents tab users can initiate a new query to retrieve external documents for the patient by clicking ‘Run New Query’ or the double-arrow symbol.
•If previous queries have been initiated you can see the last query date here as well.
Once an HIE source has been queried for the patient, any documents returned will be listed here with some demographic information for the document.
•Clicking the blue hyperlink will open the document for review. 
•Previously retrieved documents remain visible on this list. The blue floppy disk icon indicates that a document has been saved to the patients chart.
On the Outside Records tab click the ‘Reconciliation’ link to open a new reconciliation view window within PowerChart.

Clinical Reconciliation Workflow
Clinical Reconcile can be reviewed and documented from various workflow MPages by selecting the components (Histories/Problems, Allergies, Medications).

Reconciliation is intended to be a comparison of locally charted patient information to any outside
information received via CCD and then documenting that this information was reviewed.
• Any information completed in patient intake forms or charted locally will be reviewable as “verified” local record
data in the clinical reconciliation workflows.
• Any patient information from outside sources will be presented as “unverified” data from outside sources.
• Outside record data is visually designated in the chart by a purple diamond icons.

Allergies
The ‘Originating Source’ tells you where the data came from, information can be linked to multiple sources denoted by “+ X More” after the most recent source is displayed.
Sources displayed are:
•‘Patient’ if data received by the patient on a clipboard from the patient portal
•‘Local Record’ if it’s native to your patient chart
•Otherwise it will display the name of facility the data originated from

To reconcile information, select one of the unverified medication allergies, once selected a slide
panel will appear where you can add, add with changes or discard the allergy.

Duplicates can be discarded or verified. If verified the allergy will show with multiple originating
sources and related sources can be viewed in the slide panel

Once you’ve added all desired allergies to the patients verified list, click the refresh button to update
the view and you will see the allergies you added have dropped down to the verified local record data
section. 
• Selecting the ‘Complete Reconciliation’ button will document that the Allergies component of clinical
reconciliation has been completed.
Once the allergies are reconciled you will see a reconciliation status stamp under the MPage
component.
• The reconcile button will re-open the previous view and allow you to review any remaining unverified data.
• If any unverified data remains it will be viewable on subsequent patient visits as well.

Reconciling from the histories component is the same process as allergies, simply navigate to the
histories section of the MPage and complete reconciliation for histories as we did with allergies.
• An Incomplete status will be displayed in red until the individual component has been completed.
• Medication History workflow is also consistent with allergies and histories workflows, except that
you will select ‘Complete History’ instead of complete reconciliation.
